چکیده

IN THIS ISSUE of American Journal of Physiology-Heart and Circulatory Physiology, Chauhan and colleagues (5) have measured dispersion in ventricular repolarization time (RT) in patients with cardiomyopathy and an ejection fraction 40%. Such data are sparse in humans in general. The measurements have been made at the right ventricular (RV) endocardium and at the left ventricular (LV) epicardium by a catheter in the great cardiac vein. This study by Chauhan et al. (5) is one of the very few with data from the endocardium and epicardium in the same patients. Also, the patients were conscious. RT dispersion was in the order of 10–12 ms/mm in 12 patients in which either ventricular tachycardias (VT) could be provoked or in which there was positive microvolt T wave alternans (TWA). In 10 patients without VTs or TWA, RT dispersion was limited to 2–3 ms/mm. A similar increased dispersion in activation-recovery intervals (ARI; an index of local action potential duration), but not dispersion in activation times, was shown to underlie the difference in RT dispersion between the two patient groups.

Heart repolarization changes after anthracycline therapy in the children with cancer

Background Anthracyclines are important components of many chemotherapeutic protocols. The present study aimed to evaluate the repolarization changes in electrocardiography (ECG) which may predict drug induced arrhythmia. Materials and Methods In this cross-sectional study, the recorded ECGs were assessed for QT dispersion (QTd), QT corrected dispersion (QTcd), T peak to Tend dispersion (TPe...

CORRECTED QT DISPERSION IN CHILDREN WITH ISOLATED UNCOMPLICATED MITRAL VALVE PROLAPSE

Mitral valve prolapse (MVP) is a relatively frequent valvular abnormality. In both children and adults with MVP, an increased incidence of ventricular arrhythmias has been reported. QT dispersion, defined as the difference in duration between the longest QT interval and the shortest one, for a given set of electrocardiographic leads has been proposed as a sign of regional difference in card...

بررسی تاثیر اکسیژن درمانی شبانه بر روی QT dispersion در بیماران مبتلا به نارسایی مزمن قلب

Abstract Background: Nocturnal hypoxia is an important factor increasing the risk of mortality in patients with chronic heart failure and is associated with atrial and ventricular arrhythmias. QT dispersion, an index of myocardial electrical activity, is used as a prognostic tool in determining future malignant arrhythmias and sudden cardiac death. In the current study we investigated the effec...

Pro-arrhythmogenic effects of CACNA1C G1911R mutation in human ventricular tachycardia: insights from cardiac multi-scale models

Mutations in the CACNA1C gene are associated with ventricular tachycardia (VT). Although the CACNA1C mutations were well identified in patients with cardiac arrhythmias, mechanisms by which cardiac arrhythmias are generated in such genetic mutation conditions remain unclear. In this study, we identified a novel mechanism of VT resulted from enhanced repolarization dispersion which is a key fact...

Increased ventricular repolarization heterogeneity in patients with ventricular arrhythmia vulnerability and cardiomyopathy: a human in vivo study.

Increased repolarization heterogeneity can provide the substrate for reentrant ventricular arrhythmias in animal models of cardiomyopathy. We hypothesized that ventricular repolarization heterogeneity is also greater in patients with cardiomyopathy and ventricular arrhythmia vulnerability (inducible ventricular tachycardia or positive microvolt T wave alternans, VT/TWA) compared with a similar ...
